SNT Pooled Trust
People who are disabled of any age may enroll in a pooled “Supplemental Needs Trust” sponsored by a non-profit organization such as the trust administered by New York Relief for Elderly and disabled (NYRED) .
The Trust is open to any person who is “disabled” as defined by the SocialSecurity laws. When the client joins this Trust, the client agrees to deposit with the Trust eachmonth the amount of their “spend-down.”
Once the Trust documents are signed, and thelocal Medicaid program approves the client’s enrollment in the Trust, Medicaid will changethe budget so that the client has NO SPEND-DOWN. The client requests the Trust to paycertain bills, such as rent, mortgage, electric, etc. from the money the client sends in each month
CDPAP
CDPAP - A Consumer Directed Personal Assistance Program for people who require personal care assistance.
CDPAP consumers assume a number of the responsibilities generally handled by traditional home health agencies. The person becomes the aide's direct supervisor and handles recruitment, selection, hiring, training, scheduling, supervising and terminating the aides. NYRED assists consumers and trains consumers and aides on how to keep aides time records . The Agency assists participants with recruitment of aides and problem solving.
People who are interested in Self-Directed Personal Assistance Services must be able to direct their own personal assistance services or must have a family member, friend, or agency provider who is willing assume the responsibilities of the self-directing participant.Individual Eligibility:
To be eligible for this program our staff assist you in applying and being approved through Medicaid or Managed long term care receiving, for personal assistance, by the Department of Social Services. You must be eligible for Medicaid; Our staff can explain the program fully and assist you in identifying a person (aide) suited to your needs and willing to assume the responsibilities of the program on your behalf.
